Dominican Republic Tourist Visa

A valid passport must have two blank pages and be valid for at least six months.
Application for Visa completed and signed.
A pair of recent images with a white or plain backdrop.
Travel Documents Hotel reservations, travel schedules, and airline tickets.
Travel medical insurance
